Many people who have a disability rely on others to support them to get the support services they want and need. The people who support a person with a disability include:

• Nominated Representatives,
• Advocates,
• Professionals such as:
  • Social Workers,
  • Case Workers,
  • LACs, and
  • Support Coordinators

Once the form has been submitted, the Management Team will assign the referral to a Manager, who will then contact you to discuss the referral in more detail.

Note: A referral does not guarantee that Allevia can provide the support being sought, but if we can't provide the service we will assist you to identify appropriate services to approach who can.
